Health Action Plans/Physician Orders may be faxed to (812) 247-9913 or emailed to nikki.brett@shoals.k12.in.us
Healthcare Plans are required to be updated yearly and must be signed by a physician. These may be faxed to Nikki Brett or submitted to the Nurse's Office in-person.
